[Histonet] Labeling Error Policies

Our laboratories are currently working on policies to address specimen identification/labeling errors.  This is to be a laboratory policy and will include pathology.  Would anyone be willing to share policies that they have that address this and any punitive action that is mentioned?  I know this has been discussed before, and continues to be a hot topic.
