[Histonet] Cost Per Test

Hello My Histology Friends,

I have been asked to get a cost per test in histology for Level I, II, III, IV, V and VI.  Do you guys have a format or can give me some idea how to start this in an easy way.  This seems so complicated to me.  Thank you so much for your help.
