Our staining was also light.  I called CAP and they said to leave the
result area blank, mark the bubble for "unable to analyze  (11)", and to
write an explanation on the last page.  My pathologist is questioning
why we wouldn't mark "specimen unsatisfactory (33)."  What are others
that had light staining doing and/or what have you been told by CAP?
CAP also told me that they didn't have any replacement slides to send
out.

to contact the CAP to document the problem and request replacement
slides.

Cap validation Her2 slides we got one slide stained great, the other one
did not, controls and cases all stained good. We use the Dako system.
