[Histonet] Corrective Action Form

I'm looking at it from the standpoint of an inspector - we had a problem and what steps did we follow to correct it and prevent it from happening again.  I'm not planning on using it as a punishment for anyone.

Probably many of us have but it would be better if you consult with your HR Dept. because corrective actions could lead to some type of "punishment" and they have to be approved by the HR Dept.

	We recently had a false positive result on one of our immuno stains.  I
	need to write up a report indicating the problem and what the corrective
	action was.  Does anyone have a standard form that they use for
	Histology problems and resolutions that they would be willing to share
