[Histonet] Dual Esterase Staining

Happy Friday Everyone!
Our HemPath Pathologist is having us do the Dual Esterase stain on blood smears in our histology lab.  I have a few questions regarding this stain:
1.  Does anyone know what the CPT code is for performing this stain???
2.  Will anyone share what they charge the patient for performing this stain?
3.  Does any company other than Sigma sell a kit to perform this stain and how much does the kit cost???
