[Histonet] CPT questions

1.	Does anyone use the 22 modifier when billing for the Hercept
test?  It was recommended that we do this - but our billing person is
entirely opposed.  
 
2.	When billing an 88342 - do you bill different amounts based on
the actual  cost of the test, or do you bill one amount for the CPT
code?
