For all the histotechs who work for government funded research locations.

What information do you request from researchers before performing work for
them?

Do you have any specific forms that you require them to fill out prior to
doing the work?

Are you required to keep a laboratory notebook and if so what information
do you record in the notebook  (obviously any lab calculations are
necessary but what do you put for basic tasks like tissue processing,
embedding and microtomy)
