[Histonet] Cytology QA form

Curious what everyone uses for their non-gyn QA/QI form.  Just came into this histo lab, we have no Cytotech’s, Pathologists screen/read their own slides. Non-Gyn’s only. 

Wondering if anyone is willing to share their QA sheet with me to give me an idea on how to   update our form.  I know there is a calculation that is used. Our pathologist do not use this. 
# of slides divided by 12.5 multiply by 60.
