[Histonet] erroneous results

1.  Remove the result, possibly replace with a phrase similar to "Results previously reported did not belong to this patient"-- to some extent it depends on the capability of your EMR/LIS
2.  If unable to remove, then amend with same phrasing listed above, but attempt to wipe out all of the data reported incorrectly, to prevent anyone in the future from using that result for that patient.
3.  Depends on the EMR/LIS, may need to leave a trace, but make it as hard as possible to find it.
4.  Not familiar with our Pap correction process.

Could you please share your process for erroneous results?  Scenario:  Specimen received, processed and reported out.  Physician office calls to say that the specimen was labeled with incorrect patient information.  1. Do you remove the results? 2. do you amend the report? 3. do you leave any trace?  4. is your process the same for surgical specimens as it is for GYN paps?
