Concerning the use of White Out on anything considered part of the
patient's permanent record.  This actually is LEGISLATION.  It is
specifically mentioned in many state codes, and is part of the federal
laws concerning medicare and medicaid fraud. See reference below.

It may not state it directly in either CAP or GLP guidelines but in a
GLP compliant lab it is considered an industry standard not to use white
out.  As a GLP compliant lab we have an entire SOP devoted to data
entry.
