[Histonet] Specimen Labeling

Happy Friday!
 
I'm curious about what the majority of labs accept for specimen labeling, in addition to the usual, ie: name and  medical record number/PHN. What is your policy concerning specimen type/source do you require this to be on all specimen containers as well as paper requisitions? 
Is this a requirement for multiple specimens on the same patient only? or do you require it for single specimen as well?
