Just in the process of starting a new AP/histology lab myself, and what I used as my main resources to develop the quality metrics, QC and QA were:
the CAP/ASCO guidelines, CAP checklists, HistoQuip studies, "Quality Management in Anatomic Pathology, ASCP publication, 2003 ( forget the exact title), CLSI publications "QA for Design Control and Implementation of IHC Assays, 2nd edition, CLSI "Fluorescence In Situ Hybridization, methods for Medical Genetics, Approved Guideline".
United the standards in the SOP's and LIS documentation/reporting. Overall, I tied them to measures of timeliness, quality of histologic preparations, test phases and benchmarks, competency.
Monthly quality meetings, "occurrence management" with process control ( RCA, etc), Pathologist review documentation.
