[Histonet] Facility Name on Slides

Does anyone have any regs that you can point me to for both Joint Commission and CAP which details definitively whether or not a facility name is *required* to be included on slides? 

I know it's a good idea, the larger question here is whether or not there is a specific regulation for it?
