Our reagent labeling policy is this.  When you prepare a reagent for use such as a buffer.  The type of buffer is the first line, with the pH.  The second line tells what date it was prepared, if there was an expiration date and the third line is the person who prepared it.  When we receive new chemicals, dyes etc we label them with the date that they were received and the initals of the person who received the order.  I hope that is what you are wanting to know.  We also designate on the first line whether it is an aqueous solution or an alcoholic solution.  If we purchase the solution ready made we have what we call a working solution bottle of those reagents that can be reused which are labeled with the solution, where it was purchased, when it was received, when it was opened.  When to discard it, if known.
