Paula,
we don't perform frozen sections, so we don't have a cryostat and frozen
staining setup.
    We do have a control block log for immunos and special stains. We have
it set up this way
DATE   SURG #  STAIN  PATH DATE TESTED

When the pathologists gives a positive case we just write it in the book.

Every case that has a special or immuno has a sheet to go with it. It has
the DATE  SURG # STAIN TECH DR
REMARKS COMMENTS

My medical director signs off on all temp charts, maintenance schedules QC
sheets at the end of the month.
