VWR - They're called "PolyPaper Laser Labels, Catalog # 51280-116.
They're actually a Nalge Nunc product (6314-0010)
These are 7/8 x 7/8" labels that fit up to 5 lines of text nicely.

on individual sheets that fit in a LaserJet printer?
