For us, unexpected findings are noted in the report as such.  The pathologist calls and personally notifies the surgeon.  The date/time and person called is noted in the report.

Example:
NOTE: Unexpected findings of  XXXXXXXX discussed with Dr XXXX on 7/31/2015, 1115.
