With HIPPA privacy rules, patient identifies should be carefully considered.  For the "time out" procedure for patient safety, during the FNA process ,we ask the patient to provide DOB and name only.  For the slide labeling part of the process, what two patient identifiers do you all annotate on the patient slides.  The same aforementioned or something else. The impasse is space on the slide.  is patient first initial of last name and last four acceptable?  Or should we stick with name and DOB for consistency of policy. Or does it matter?
