  I used to have "competency check lists" and "standard of performance descriptions" for each mayor task. Along the year I used to observe the activity of each HT in each task and evaluated them against those lists.
  The deficiencies I found were documented and used for the individual PI of each HT, which went to their personal file.
  When during the CAP inspections I was asked about this aspect I showed them the PI developed for each HT that required a re-training on any given task.
  They were pleased with this approach to documenting the proficiency in completing histology tasks.

I have spoken with CAP about the HistoQIP program and they consider that an educational program, not a proficiency program. Just wondering what other options are out there if any.
